The following steps will help make the first step an easy one
1. Call our office (920) 882-6610 to arrange for an "initial evaluation".
2. If you are using insurance, have your insurance card available as you will need to
provide basic information in order to reserve an appointment.
3. We also highly recommend that you contact your insurance company. Use the form
"Insurance Questionnaire" as a guide when calling.
4. If you are privately paying (no insurance), an appointment time will be set for you.
If you have registered as a private pay patient, payment is expected at time of service.
